Quoted from HowardR:Does the Game Over (Q) relay activate when this happens?
Quoted from NickNitro:Hi Howard, I just checked and yes it does in both instances
Since the Game Over (Q) relay activates when it shouldn't, the next step is to find out what's causing that. Under most conditions the cause will be one of these 2 switches: (1) this make/break switch on the Hold (R) relay, or (2) this Motor 4C switch. If neither one is obviously gapped too close, put pieces of paper or other small insulators in them to find which one is at fault and then we can go from there.

Definitely an actual relay you can find in the list of relays at 11E on the schematic. According to this picture of the inside of the 2 player Top Score, the EE relay is in the backbox.

Quoted from NickNitro:why it doesn't always advance the ball count, only advances about 1/5 times. I'll try and figure that out on my own but if you had insight on that, it would be great!
If the Add Player Unit (P) relay doesn't activate when this happens, check the Trough switch between the Outhole and the Shooter lane.
