That might explain why the Out Hole relay doesn't fire. Have you looked into why?

Once it fires the Ball Index relay should hold until the 6D Score Motor switch opens while the Out Hole relay is active.

Yeah, it sounds like you're not counting the bonus down to zero, which needs to happen before the Out Hole relay fires and the ball is returned. It works after the first ball because your bonus is (incorrectly) at zero (Bonus Zero relay).
When the Out Hole switch closes the Bonus Score relay should fire unless no points have been scored (Ball Index relay) or there's no bonus to count (Bonus Zero relay). Does your Bonus Score relay fire when the ball drains?
I think the Bonus Unit should take one step whenever there's no bonus (Bonus Zero relay) and the Out Hole relay fires, so you always start with a bonus of 1000.
