Here is your game over relay, it is the only relay with two coils attached. It is located on the mech panel, on the bottom of the machine. You need to check MB switch D. It is most likely not opening when you start a game.

A MB opens one switch and closes another when activated. N.O. Is normally open, meaning, before the switch is activated, it is open.
