Finally got it figured out. The OPB804 Opto must not be a perfect match for this game. I had to replace all my original optos with OPB804 as all of the originals were faulty. To rid the switch matrix issue I ended up having to add a diode at the opto board for each optos column wire. Once that was completed all switch matrix issues resolved and the game is now 100% playing!

Fantastic troubleshooting. Way to stay at it.
