A scope on the output would tell us a bunch, like is there an open diode (ripple will be really bad) and if you are getting lower than normal 5 VDC, it may be acting like a half wave rectifier which would pull 12v down to about 10 or 11 and 5v down to 4.8 or so.

Replace the bridge rectifier, it is probably exactly what is wrong.
