One of the connectors looks like a Molex 1625-2PRT - .062" series 2-circuit connector.
The upper-most connector is some type of IDC connector - 2-circuit. You'll need a punch-down tool to properly use these type connectors or you can replace them with more reliable crimp style connectors.
You can find them at Digikey or Mouser
The little LED array - I have no idea who has them.
