    Oops, I see the post until today.
    Imho there is a fault in your layout. Pin 8 must be connected to the ROW B,4,5,6.

    In my layout, you found on the pinrepair site, I indeed used 2,54 spacing, as I wanted to keep my keypad as small as possible and I built an adapter cable from 2,54 to 3,96.

    Final design (with 3,96 connector) could look like this:
