Had you recently cleaned or waxed the playfield? Mine did this a number of times right after I cleaned it last time. It is frustrating as it would only do it on a really clean fast shot up the ramp.

I was having the same problem and my issue turned out to be a dent/mushrooming on the end of the shooter lane from balls smacking it. Filed it smooth and it has worked almost perfectly ever since.
