1. One of your Lock optos is out, or registering as closed, making the game believe there's a ball there. Switch test will lead you in the right direction there.

2. As LTG says, the second flip is programmed into the ROM - unless a switch is hit relatively quickly after the first 'dump', the software will dump it again in case the ball wasn't seated properly in the bridge cage. For the growling, adjustment is needed. Search here on Pinside for Bridge Diverter sticking, and you'll come across several posts with different things to try. (From adjusting the coil, to adjusting the connecting rod - it can be several things causing it to 'stick')

It is not a 'searching' issue. The software is seeing the lock area occupied somehow. Could be an issue with the opto board (since it happens on power up, bad cap causing noise in an opto signal, not not giving an opto proper voltage when first getting powered up. +12v taking a while to ramp up, etc.)

Could be either. Also could be the large, 1/2w resistors cooking through the board, if your opto board is original. (If you have access to a second opto board, swap them and see what happens. Would save you some trouble..)

I'm not saying this is definitely it, but I know how that software works. Could also be matrix issue - the clock is being tested at this same time, and it's possible that some feedback from the clock's rows are going into the switch matrix (bad diode on clock board?) causing it. However, if this were the case, then the game would try firing that anytime the clock was moving while the lock trough was supposed to be empty. Since you didn't say this was happening, it's likely not the case..

(Also, well, what revision ROM are you on?)
