Sounds like you have several different issues. I would focus on one, and then work your way around.
First things first - go into switch test and make sure ALL SWITCHES work as they should..

I would be extra dutiful on checking switches..
Make sure none are Active ("(A)" displayed on them) when they shouldn't be.
TZ has rather robust software, and will try to compensate for broken or erratic switches. (Ref that other thread where 'fast lock' wasn't starting because a trough switch was caught.) So, something as simple as the 'Camera' switch being flagged as bad can cause the slot machine hole to award a camera award when lit.
