1 is def power fluctuations. try a different power conditioner or something.

i know on my cyberpower the voltage was ok but you could hear the whine when it switches over, I know it was not a full 60hz sine coming out of it, it was a bit low, which is probably what the pin uses to detect between a 50hz / 60hz signal (ie: usa v rest of world).
