Ignore the "grounded to flips" and think of these as nothing more than direct ground switches.

There are "row" diodes on the fliptronics board, but these don't serve the same function as switch matrix diodes; these serve the same function as the row diodes on the CPU board.

There are many reasons why this could happen, dirty optos, bad ribbon cable, loose ASIC, failing LM339, low 12V...

Yes, by design, a failure of any of the optical switches would effect only that one switch.
