I had same exact problem and did the exact same thing. New board, same issue.
With the help of a super smart pinhead, we stepped through the power, from the opto board all the way back to the power supply. Repinning along the way. Ultimately the issue was cold solder joints on the power supply board. Once we reflowed, it started working like a champ and is still running strong.
