Did the opto come out of the bracket somehow? You should be able to see in there to see the opto on either side. Transmitter and receiver.
FYI- infrared light from the opto is invisible to the naked eye, but can be seen if you look at the transmitter with a most phone cameras. Its a good way to sanity check. It will look pink/faint purple.
Took some pictures. I cant see pink hue. Also checked connectors on io board...reseated still nothing. Tried looking for the optos but tough to see if they are there are in the right position or if they have come undone. how easy is it to unscrew the theatre housing and check under?

Hey man! I can see the issue. Right side opto looks like it came out of the bracket. You can see it in the reflection off the playfield clear coat in your bottom picture. You'll have to take the theater off, and that plastic, but then you should be able to get in there to solve the problem.

Turn game off. Lift playfield and pull out and rest on front of cabinet.
Remove clear plastic over pop bumpers, set to side, some nuts or screws on theater - set aside.
Then you can see the two optos, one each side. Stick back in and a dab of hot glue. Reassemble.

Even if it is embarrassingly simple. Helps the next person doing a search on a similar issue.
Solution - remove the 2 small screw holding the mini playfield over the bumpers. Unscrew nut bolt on top of front plastic. Remove bumper cap beside theater. Raise theatre slightly. Glue opto back in place. Put everything back together. Play for 5 hours in a row.
Thanks for posting this! Had the same issue except in my case it was the opto receiver that was hanging loose. I noticed the transmitter has a glob of glue on it. The receiver has none. I imagine it is supposed to? I will have to pick up a hot glue gun.
Nevermind... receiving opto has a black plastic 'cap' over it. That cap had popped off and was sitting in the bottom of my cabinet. Had to remove theater, insert receiver into the little hole and snap the plastic cap back on. Working great again!
