Use your fingers to tap around each opto while in switch test. Also tap the opto board around the connectors. If you haven't already, reseat the connectors on the opto board.
It's likely just a bad opto or flakey connector. Those transceivers can get real sensitive when they go bad.

SAM games use transceiver optos, meaning both optos for a switch are identical. No idea why that fixed your game, but it wasn't miswired or mislabeled from the factory.
