According to the readme file, the game rom should go in IC14.
This is the special place to put an eprom.
The other positions are for smaller sized roms.

No, but only if it is a 2716 eprom.

Yes, you need the yellow roms too.
That's the operating system of the game.
Do some reading on the http://www.pinwiki.com for more details.
