I chased this issue for a short while on my Taxi after I bought it. I found broken wire leads hidden within the black opto case close to the lens. I simply replaced the opto's completely and my intermittent problems were gone. You can re-enter a switch test, drop the target and gently wiggle the board, and/or then the opto to see if you get a sporadic reading, that's how I located my failure.
