In addition to the voltage and ground, 1 of two driver lines needs to be grounded to make the motor spin. (One is for clockwise, the other is for reverse.)
If you manually short the driver lines to ground, the motor should move. If it does not, check the motor plug on the board. You should get 12v one way, and -12v the other way, and nothing with none of the driver lines grounded.
If you do not get voltage out, then something on the controller board is shot - likely the LM6203. If you ARE getting voltage out, then it's something in the motor or it's wiring.
