I just fixed similar issues by repflowing the entire opto board. I highly suggest it. I had the same issues.

I chased this one for a year. Finally I bought a rottendog driver board and that was the end of the problem. I would suggest either replacing with a rottendog board OR sending your board to a reputable board repair firm and have it completely tested.
