This is a Pinnovators PINsub SAM subwoofer interface board for Stern SAM Systems. It's basically their PINsub board standalone, without the 12ft 3.5mm male-to-male cable, or the 3.5mm-female-to-RCA-male adapter.
I installed this on January 21, 2021, and uninstalled it on February 19, 2021, because after trying the PINsub and ABSOLUTELY loving it, I upgraded to their PinPAC MAC headphone kit, so this is basically an extra board that I'm not using.
It's in superb condition, and I highly recommend it. All you need an an audio cable to connect it to your subwoofer connection of choice.
