Chris, I agree, a form is the way to go. I made one for when I was doing pinball shipping. Worked out great. I think I would have them print the form out and include it in the box of parts. You may also want to submit the form to the website so you know what is coming. I would think at a minimum you want, customers name, date of shipment, list of parts, and what to do to each part, color, and a return address and phone number. In my case I would not confirm a shipment until I had all the information I needed. Sometimes getting the information you need is like pulling teeth. Good luck
