The guy must have bent his finger, and was pinned by what was left of the flipper button, or the flipper switch. Fingers are normally tapered, seems odd that a small screw driver was not used between his finger and the opening.

The same exact thing happened locally here in 2005. I had a TSPP which I sold to a local operator who put it in a movie theatre. The right flipper button came loose and someone stole the button. Some girl then played the game and to operate the flipper she had her finger in the hole, pushing the leaf switch. Unfortunately, her finger got stuck. If you think about it, it's the leaf switch is what traps your finger and trying to pull only makes it dig into your finger even more. So my friend had to go to the theatre, open the game and free her.
