Monitors are named for the chassis/pcb, not the glass tube. A particular monitor could have been sold with several tube manufacturers. That is a wells gardner k4800 chassis, which is the 13" variant of the extremely common 19" k4900.
That black piece in the neckboard socket should remain on the tube to protect the alignment of the pins. Not a big deal, but be careful when reinstalling.
