You are right about a transformer never failing.

When I first starting working after college I saw a transformer fail of natural causes.

A wise old power engineer told me to remember that day because I'd never see another.

He was right. As long as they were cooled right (when needed) and had the proper over current protection I have never seen one fail by natural causes since. Buzzy or noisy, but never fail.

30 plus years.
