Meteor Switch issue

I have a switch issue I just can't seem to solve. The switch test is fine for all switches, but during gameplay the top target of the #3 Bank does not register. It is the only switch that does not. The other kicker is that when it is down / switch is closed it knocks out another column of switches on the matrix. If you manually open the switch by lifting the target up, they all start working again.
Any ideas on this one?

check the diode on that switch..it could be bad,faulty.

I changed the diode, unfortunately it did not solve the issue.

is the wiring on that switch soldered on in the correct order?
