I know this isn't a repair thread, but I can tell you exactly what is wrong - you have a switch matrix error. You have 1-2 switches stuck, and when the ball is out of the outlane hole, it completes a "ghost square" in the matrix to stick yet another switch.

Put the pin in diagnostics and switch edge test. See which switches are shown as closed. Look at them on the playfield and "unstick" the stuck ones.

I had the exact same symptoms. The stuck switches were one on the ramp, and one under the cyclone ramp due to a broken rubber.
