I ran into the same after a recent flipper rebuild. Ultimate “solution” was simply to tighten the crap out of that pawl (despite all the warnings not to).

Best I can figure is that the metal gets deformed when tightened, so any subsequent adjustments require going tighter than before.

Note: I’ve only been a pin owner for a month or so. Take my words with a massive grain of salt

Also keep in mind Vid's advice that even if the gap between the sides of the pawl end up touching, you can pull the bolt, shave some off the inside, etc:

File, use an air cut off wheel or Dremel to grind the gap slightly larger. It won't take much.
