I'm going with Gigi, but I'm not totally convinced.
GTB 12/1963. The episode was 1965.
Look at the side of the main cabinet. A few big circles with horizontal stripes. Top stripe just above top of leg.

Also, big blocky areas where head meets cab. Blacked out in the center. Fat, horizontal white stripe across the bottom of head.
And left vertical side of head transitions from light to dark as you go down - due to skinny white tapers?
But the artwork in the backglass doesn't really match in terms of dark areas vs. light areas. I think the lower right bulb at the clown's face must be out or else that region would be illuminated. The other bulb illumination areas do seem to match up.
OK, time to stop - before I go insane.
Squint while looking at the Gigi head shot and all of the dark areas of the TV shot appear. I'm convinced. Well, about 95% convinced. But what else could it be

Just watch the show not on youtube and you should be able to see it much better. I was half asleep, and the scene came and went so fast, it was sort of a blur, but not this blurry.
It does look a lot like Gigi, but when I first saw it I wasn't sure it might not just be a custom job like they have used in the movies.

This codger doesn't even have DVR. I just watch what comes on. And never really feel like I've missed something or the need to record it. This show will be on again for the millionth time someday. Someday I might even make it thru a whole episode again.
Fun fact- the last Mason episode was in color. The only one. Some young hoodlums stripped Perry's car down to nothing in less than five minutes.

I'm 100% sure it's Gigi now. Upper left clown has a white glove. White spot in same area on b/w photo.
The clown below him has a lighted head. Same spot is white on b/w photo. I can even see "Marilyn's" outline now.

Peter Falk is playing a Bally Casino and as far as I remember a Gtb Flying Carpet as Columbo in the episode "The Conspirators"
There are a couple of games that can be seen in The Hustler with Paul Newman. I remember seeing some cabinet bowlers in a bar and in the bus station.
It's been over a year but I saw that episode of Perry Mason again last night and I made sure to pay more attention this time. Even on a 40 inch flat screen it was grainy and kind of hard to tell, but I can say with 99% confidence that it was indeed GiGi.
This time I saw the Perry Mason episode The Telltale Tap on my 52 inch Sony and I could actually read the word "Gigi" on it. Don't even need glasses.
I guess that settles it once and for all.
