Dirty Harry Switch Matrix Issue

So, I'm thinking this is a board issue... But here goes. On start up, I get 1 error - Right Ramp Entry (#51)... I go to test menu, and when I depress the switch, it lights up switch #41 and #31.... The pin has not been hacked or anything... I should mention prior to all this, I replaced the switch to clear the error and it did nothing different. Thoughts?

Did you wire the switch correctly? Diode installed? Diode installed in the proper direction? Compare your work to what the other switches look like.

Times I had a switch issue caused by something on the board, 1%. Times I had a switch issue caused by something on the switch, 99%.

Follow-up: it was a wiring issue... not sure how it happened since it had the error before I bought it but I actually have two Dirty Harry pins and was able to see the 2 wires had been swapped when I compared. So worried about the diode direction
