The issue is most likely with trough 3. It could be a reversed diode, diode shorted to ground or diode lead shorted to switch lead.

As DumbAss said you can also confirm the board is good with the post he linked. If it's the board you would have more problems, but sometimes the issues can be less than obvious.

White wire should be towards the non-banded side of the diode (technically the diodes should be reversed). Also you really need to clean up some of that soldering and the extra wire/leads sticking out from the solder joint. That's an accident waiting to happen.
