Manufacturing defect on earlier machines that causes a SIM card shot to roll over to the BOB opto, making the game think you scored a BOB shot. Under the game you will see where the SIM drop meets up with the BOB subway. Fit a shim (anything will do but TAF vault shims work great) in so that the SIM drop is clearly lower than the subway where they meet. On incorrect games, they are at the same level.

Mistake in assembly where they wired [usually two] lights to the wrong spots on the BOB sign (issue on a many of these games). Easy fix...The Extra ball light will be orange, multiball red, and Crazy mode blue. So when they are lit during game play you can simply pull the wrong ones out and stick them in the correct spot.
