I'm experiencing, and can replicate it 100% - a 'lock-up'. Where all the lights of the playfield light up, all score displays reset to 0, and no response from flippers. I can restart a new game by hitting the 'start' button as normal, and the game seems to play normal afterwards. Score from prior game is lost.
The way I replicate the problem is by, in a game, hit both flippers at the same exact time.
What is this? Should I just replace the diodes on the flipper? Something more involved? Known problem?

Diodes on the flipper coils can't hurt. I would also replace all the header pins on the power supply board (not just reflow them). Reflow the pins on the wafer connectors (the rectangular connectors near center bottom) and inspect the pins and sockets for loose, worn pins.
