Try new T nuts - any hardware store has them. Bring one of the bolts with you to make sure you get the right size.

Make sure the pointy ears are biting into "fresh" wood - in other words, don't let the ears go into the holes that were used by the old T nuts. Tighten the bolt, and the T nut should snug into the wood, and not fall out again.
