The insert location issue could have started with your scan. I have a super high end scanner and it is still off a fraction of a percent in one direction and another fraction of a percent in the other. This adds up when you stitch something the size of a playfield.

I now scan carefully placed precision rulers along with any artwork I scan then adjust my scan in software before proceeding/stitching.

And the printer could also be doing the same. Don’t be surprised if you absolutely nail it in the kinkos printout then your off a bit when you get your final produce from another source.
