Didn't Bally release out a table called Balls a Poppin back in the 50s, which had a crazy multiball?
There's also 1972's Fireball, which had both 2 and 3 ball multiballs.

Even the regular game can give you an instant multiball, if you take advantage of the extra ball button. It will instantly start up the 4-ball Judge multiball.

That's only true on their dot matrix tables. Their alphanumeric tables were quite challenging to start up the multiball mode.
