Anyone have a picture of the back of the monster slide plastic? I have the plastic and two cut wires. Not sure what socket/socket bracket goes on the back and can't find a good picture.

If you are talking about the ramp behind the board then...
No picture but the two parts of the ramp slide together (one overlaps the other) and then there is a screw that goes through both pieces into a L-bracket that is mounted to the board.

Or maybe the attached pic it what you needed. My wires could be wrong since I had that problem but not sure if this pic was before of after I fixed them.

Great. Now I need to find this single lamp pcb 5768-12312-00. Can't find it anywhere.

Looks like a single 555 board from a playfield insert would work.

It would.. if I could find one. Anyone point me in the right direction. I've looked on:

Bay Area
PB Resource
Action Pinball
Pinball Life
Great Plains

I can't find it on any of those. I've emailed them to see if maybe they just don't have it on their website but...

The board missing or damaged? If it was damaged I would just jump out the traces and if its missing you could make one with a breadboard.

Missing entirely. I could make one but I'm a stickler for original parts if I can find them.

Screws holes don't look like they'd line up.

worst case I get one of these bad boys and get to moddin.. $1.25 better than $8.75 if it's not gonna be OEM anyway!

Yeah, pretty much my thought too. Although I'd probably go with a bayonet base instead. (ala "LOCK" in SS.)
Hopefully your Pinside connection can get you the original part so you can rest easy.

Try this site, I have seen the lamp holder that goes in to the PCB, they may have the PCB as well.
