    OP, if you unplug it and plug it back in when it's on and working correctly does it stay shut off like the problem you are describing? I wonder if it's some kind of toggle

    The one on the bottom right? Isn't that just another output? (On the end of the red and whites or just the white, whatever it is)

    What's the second output for?
