    I'm pretty happy with this now

    here's a printable version with tiny cut lines

    on the original, there were a lot of places where the fonts were changed for no apparent reason, giving a very uneven look and feel
    on this version, I cleaned that up and repositioned some of the text in each section to help balance it a bit better

    If it's just printed on card stock - get at least 2 just in case. If not 4.

    yep, it appears to have been originally printed on a white faced heavy card stock

    the image is the correct size for those that want to get it printed themselves. you just will have to cut out the "white" shape

    added a printable version to the above post with tiny cut lines. let me know if the cut lines need to be a bit larger after printing it out

    Just send a file to a local print shop and tell them to print it out on card stock. I do this all the time with my local FedEx Office place. I'll send them the file and also note what the finished size is supposed to be just to make sure once they receive the file nothing changes. It's not expensive at all and having an actual print shop do it is better then what one can do on a home printer.

    the actual size of the documents are:
    22-15/16" (w) x 4-1/2" (h)

    there is a 1/2" border around the actual instruction card (which was black on the .jpg version of the image)
