Yeah, I was there at the "unboxing". I left right before the issue happened. :/ No shaker on this one AFAIK.

I did reseat the two trough board connectors, along with the Z connector from the trough, the phantom switch closures came right back, so it has to be the opto boards.
I can attest to that. We went over the entire pin and checked all connectors before it was even plugged in.
